4. Internship Completion Certificate in original.
5. Details of Internship Completion Certificate duly counter Signature by Principal/Dirtector.
6. C.L.C in original from where you have passed MBBS.
7. Pay Online For Rs. 2000/- (Two Thousnad).
8. Proof of age - Board Certificate etc. ICSE, CBSE, State.
9. Provisional Registration certificate in Original Copy.
10. Upload Provisional Registration Certificate.
11. Upload Aadhaar Card.
12. Upload All Documents in One Pdf.
13. Maximum File Size is 5 MB.
14. The permanent address mentioned as per your Provisional registration certificate has to be filled in the Application.
If the address is to be change, then an affidavit has to be given as per the address of aadhaar card.
