1. Application Form of the Candidates.


2. Upload Degree certificate - MBBS, P.G Degree if any (Attested by any Gazetted officer).


3. Upload Previous Registration certificate - attested by any Gazetted officer.


4. Upload C.L.C of the college from where you have passed MBBS & P.G. Degree - attested by any Gazetted officer.


5. Pay Online of Rs, 2000/- (Rupees Two Thousnad).


6. Pay Online of Rs, 700/- (Rupees Seven Hundred) per P.G. Degree. (A delay fine for registration of PG degrees @ Rs. 200/- per year per degree)


7. Passport size photograph.


8. Signature. (If the signature submitted does not tally with the signature given on the previous registration certificate, registration may be refused.)


9. No Objection Certificate from state medical council from where you were registered. (not more than one month old N.O.C in original, subject to Confirmation from that Council.)


10. Photocopy of Aadhaar Card.


11. If you are registered with Bihar Medical Council, then - Postal Order of Rs. 1000/- (One Thousand Only.) for Bihar Council of Medical Registration as N.O.C fee, submit at the time of verification in this council (subject to Confirmation).


12. If you are registered with Medical Council of India, then N.O.C is not required, subject to Confirmation from M.C.I.


13. Registration Certificates will be issued to the candidate personally with signature in receiving register.


14. Upload All Documents in One Pdf.


15. The Permanent address mentioned in your previous Registration Certificate is to be filled in the application
If the address is to be change, then an affidavit has to be given as per the address of aadhaar card.


Note: After applying online you will have to come to the office with original documents for verification at the given time.
